How To Apply

Citi no longer accepts paper form for supplementary card applications. Please return your application form and supporting documents to us through Citi Mobile App ®

Submit through Citi Mobile App®:

  • Login Citi Mobile App®
  • Select your Citi Credit Card under Card dashboard, then click “Manage”
  • Click “Apply for a supplementary card”

Documents required:

  • HKID/Passport copy of Supplementary Card Applicant
  • Additional government issued identification document copy (e.g. passport or Mainland Travel Permit for Hong Kong and Macau Residents (Home Return Permit) etc.) of Supplementary Card Applicant
  • Citi Prestige Card
  • Citi PremierMiles Card
  • Citi Cash Back Credit Card
  • Citi Rewards Credit Card
  • Citi Clear Credit Card
  • Citi Octopus Credit Card
  • Year Round Dining Offers
